How this is calculated

A semen analysis (seminogram) measures several parameters to assess male reproductive potential. The World Health Organization publishes reference values based on fertile men whose partners conceived within 12 months of unprotected intercourse; the 2021 sixth-edition values supersede the 2010 fifth edition. The current lower reference limits are: volume ≥ 1.4 mL, sperm concentration ≥ 16 × 10⁶/mL, total sperm number ≥ 39 × 10⁶, progressive motility ≥ 30%, total motility (progressive + non-progressive) ≥ 42%, and normal morphology ≥ 4% using strict (Kruger) criteria.

This calculator derives total sperm count by multiplying ejaculate volume by concentration, then computes total motile count and progressively motile count. Each measured value is compared with the WHO 2021 lower reference limit and flagged as within or below the reference range.

Being below a WHO reference limit does not confirm infertility — these are the 5th percentile values of fertile men, meaning 5% of men who recently fathered children scored below them. Conversely, normal values do not guarantee fertility. A clinical assessment requires full laboratory analysis, history, physical examination, and sometimes genetic or hormonal testing. Frequently asked questions

Concentration is the number of sperm per millilitre of ejaculate (×10⁶/mL). Total sperm count is concentration multiplied by the ejaculate volume — it reflects the total number of sperm in the whole sample. A large volume with low concentration can still yield a normal total count, and vice versa.

Progressive motility (PR) is the percentage of sperm moving actively forward, in either straight lines or large circles. Non-progressive motility (NP) includes all other movement patterns. Total motility = PR + NP. Progressive motility is considered the most clinically relevant motility parameter for natural conception.

The WHO 2021 sixth edition revised several limits slightly: volume dropped from 1.5 mL to 1.4 mL, concentration rose from 15 to 16 ×10⁶/mL, total motility rose from 40% to 42%, and progressive motility dropped from 32% to 30%. Morphology and total count remained at 4% and 39 ×10⁶ respectively.
