SCORAD Calculator — Atopic Dermatitis Severity Score
Compute the SCORAD (SCORing Atopic Dermatitis) index — the most widely used clinical tool to grade the severity of atopic eczema — from body surface area involvement, six objective intensity signs, and two patient-reported subjective scores.

Severity: Moderate

Extent component (A ÷ 5)
20 ÷ 5 = 4 - 2
Intensity sum B (6 signs)
1 + 1 + 1 + 1 + 0 + 1 = 5 - 3
Intensity component (7 × B ÷ 2)
7 × 5 ÷ 2 = 17.5 - 4
Subjective component C (itch + sleep)
3 + 2 = 5 - 5
SCORAD
4 + 17.5 + 5 = 26.5
How does this calculator work?
SCORAD = A/5 + 7×(B/2) + C, where A is affected body surface area (0–100%), B is the sum of six intensity signs (each 0–3, max 18), and C is itch + sleep VAS (each 0–10, max 20). Maximum score is 103; mild < 25, moderate 25–50, severe > 50. Developed by ETFAD.
Formula
How this is calculated
SCORAD was developed by the European Task Force on Atopic Dermatitis (ETFAD) and combines three weighted components into a single 0–103 scale.
Component A is the body surface area affected, estimated using the rule of nines (head and neck 9%, each arm 9%, each leg 18%, anterior trunk 18%, posterior trunk 18%, perineum 1%). It is divided by 5, contributing up to 20 points. Component B is the sum of six objective intensity signs each scored 0 (absent), 1 (mild), 2 (moderate), or 3 (severe): erythema, oedema/papulation, oozing/crusting, excoriation, lichenification, and dryness. The sum B (max 18) is multiplied by 7/2, contributing up to 63 points. Component C is the sum of two visual analogue scale (VAS) scores (each 0–10) reported by the patient: itch severity and sleep disturbance averaged over the last three nights, contributing up to 20 points.
The final score is classified as mild (< 25), moderate (25–50), or severe (> 50). SCORAD is a clinical instrument — its validity depends on accurate skin examination by a trained clinician; it is not a substitute for medical diagnosis or treatment advice.
Frequently asked questions
SCORAD is the primary validated tool for objectively measuring atopic dermatitis (eczema) severity in clinical practice and research. It is used to assess baseline disease, monitor treatment response, and compare outcomes across studies.
SCORAD includes both objective signs and patient-reported subjective symptoms (itch, sleep loss), giving a total of up to 103 points. EASI (Eczema Area and Severity Index) covers only objective signs weighted by body area, with a maximum of 72. Both are validated; choice often depends on the clinical or research context.
The tool can help patients understand the score structure, but the intensity signs (erythema, lichenification, etc.) require a trained clinician to assess reliably. Self-scored SCORAD may differ significantly from a physician-assessed score and should not replace a medical consultation.
