How does this calculator work?
CRC = Reticulocyte% × (Patient Hct ÷ Normal Hct) corrects for anemia's dilution effect. RPI = CRC ÷ Maturation Factor (1.0–2.5 depending on hematocrit severity) further adjusts for early reticulocyte release. RPI ≥ 2 indicates adequate marrow response; RPI < 2 suggests hypoproliferative anemia.

How this is calculated
Reticulocytes are immature red blood cells freshly released from the bone marrow. In a healthy adult they make up about 0.5–1.5% of circulating RBCs. In anemia the bone marrow should ramp up production, raising the reticulocyte percentage — but because fewer total RBCs are in circulation, even a mildly elevated raw percentage can be misleading.
The corrected reticulocyte count (CRC) rescales the raw percentage by multiplying it by the ratio of the patient's hematocrit to a normal reference hematocrit (typically 45% for adults). This accounts for the dilution effect of anemia.
The reticulocyte production index (RPI) goes one step further and divides by a maturation factor. In severe anemia, reticulocytes are released prematurely and spend longer in the peripheral blood (up to 2–3 days instead of 1 day), so each one counts for less than a fully matured cell. The maturation factor (1.0 at normal hematocrit, rising to 2.5 below 20%) corrects for this. An RPI ≥ 2 suggests the marrow is responding adequately (hemolysis or blood loss); RPI < 2 points to hypoproliferative anemia (iron deficiency, B12/folate deficiency, marrow failure).
