How this is calculated

The Pearl Index (PI), introduced by Raymond Pearl in 1933, is the standard measure of contraceptive effectiveness. It counts the number of unintended pregnancies that occur while 100 women use a method continuously for one year. Mathematically it is the number of failures divided by the total woman-months of exposure, multiplied by 1200 (= 100 women × 12 months): PI = pregnancies / (women × months) × 1200.

Woman-months simply counts total person-time: 100 women followed for 12 months each yield 1200 woman-months = 100 woman-years. A lower Pearl Index means a more effective method. Well-known typical-use values (approximate, from published trials) are: no method ~85, male condom ~15–18, combined pill ~0.3–9, progestogen-only pill ~0.3–8, copper IUD ~0.6–0.8, and hormonal IUD (Mirena) ~0.1–0.2.

The Pearl Index has limitations: it assumes a constant failure rate over time (the denominator grows linearly but actual risk may decrease as less-fertile cycles are "used up"), it cannot distinguish method failure from user failure without further analysis, and studies with different follow-up durations are not directly comparable. The life-table method is preferred for long-term and multiple-cycle data, but the Pearl Index remains widely cited.

A Pearl Index near 0 (e.g., hormonal IUDs ~0.1) indicates very high effectiveness; values around 15–18 (male condom, typical use) represent moderate effectiveness. No contraception has a Pearl Index of roughly 85 — meaning about 85 in 100 women would become pregnant in one year without any method.

Perfect use assumes the method is used correctly and consistently every time. Typical use includes real-world mistakes such as missed pills or incorrect condom use. For example, the combined pill has a perfect-use Pearl Index of about 0.3 but a typical-use value of 6–9 because of missed doses.

Studies differ in follow-up duration, participant age and fertility, loss-to-follow-up rates, and how they define and confirm pregnancy. A trial of young, highly fertile women over 24 months may yield a higher Pearl Index than a trial of older women over 6 months, even for the same method. Always compare PI values from similar study designs.
