The Padua Prediction Score (published by Barbar et al., J Thromb Haemost, 2010) stratifies venous thromboembolism risk in acutely ill hospitalised patients. How does this calculator work?
The Padua Prediction Score sums weighted points for 11 clinical VTE risk factors in hospitalised medical patients. Score ≥ 4 = high risk, where pharmacological thromboprophylaxis is generally indicated. Published by Barbar et al. (2010); always combine with individual bleeding-risk assessment.

How this is calculated
The Padua score was derived and validated in a cohort of 1,180 consecutive inpatients at the University of Padua. It assigns weighted points to 11 clinical risk factors identified by logistic regression as independently predictive of symptomatic VTE. The four highest-weight factors — active cancer, previous VTE, reduced mobility and known thrombophilia — each contribute 3 points, reflecting their strong individual association with thromboembolic events.
A score of 4 or above classifies the patient as high risk. In the original validation study, high-risk patients who did not receive pharmacological prophylaxis had a VTE rate of 11.0% versus 0.3% in low-risk patients. The score is one of several validated tools (alongside Caprini, IMPROVE, Khorana) and is most commonly applied to medical (non-surgical) inpatients.
This calculator is educational. Clinical decisions about thromboprophylaxis depend on the individual patient's bleeding risk, contraindications, institutional protocols and the treating physician's judgement. The score should never replace clinical evaluation.
Frequently asked questions
A Padua score of 4 or more indicates high VTE risk. In the original 2010 Barbar et al. study, high-risk patients not given prophylaxis experienced VTE at a rate of 11%, compared to 0.3% for low-risk patients (score < 4).
The Padua score was developed for acutely ill medical inpatients; the Caprini score was developed for surgical patients. Both assess VTE risk but use different criteria and weightings suited to their target populations. Use the appropriate tool for the clinical context.
The theoretical maximum is 20 points (all 11 criteria positive: four 3-point criteria = 12, one 2-point = 2, six 1-point = 6). In practice, scores above 10–12 are rarely seen because many criteria are mutually exclusive or rarely co-occur in a single patient.
