How does this calculator work?
EASI = Σ (Intensity × Area × Weight) over head/neck (×0.1), trunk (×0.3), upper limbs (×0.2), lower limbs (×0.4). Intensity is the sum of 4 signs (0–3 each); area is scored 0–6. Total range 0–72: mild ≤7.0, moderate ≤21.0, severe ≤50.0, very severe up to 72.

How this is calculated
The EASI is the most widely used validated outcome measure in atopic dermatitis research and clinical trials. It assesses severity across four body regions (head/neck, trunk, upper limbs, lower limbs) and has a maximum score of 72. Each region contributes a weighted subscore computed as: Intensity × Area × Body-weight.
The intensity score (0–12) is the sum of four clinical signs each rated 0–3: erythema (redness), edema/papulation (swelling/raised lesions), excoriation (scratch marks), and lichenification (skin thickening). The area score (0–6) reflects the proportion of skin in that region affected: 0 = clear, 1 = 1–9%, 2 = 10–29%, 3 = 30–49%, 4 = 50–69%, 5 = 70–89%, 6 = 90–100%. Body-weights reflect anatomical surface proportions: head/neck 10%, upper limbs 20%, trunk 30%, lower limbs 40%.
Severity categories: 0 = clear; 0.1–1.0 = almost clear; 1.1–7.0 = mild; 7.1–21.0 = moderate; 21.1–50.0 = severe; 50.1–72.0 = very severe. EASI was originally validated by Hanifin et al. (2001) and is widely used as a primary endpoint in pivotal clinical trials. It should be scored by a trained clinician examining the patient — this tool implements the published formula exactly.

The intensity score for each body region is the sum of four clinical signs — erythema, edema/papulation, excoriation, and lichenification — each rated 0 (absent), 1 (mild), 2 (moderate), or 3 (severe). The maximum per region is 12 (all four signs at maximum severity).
An EASI of 21.1–50.0 is classified as severe, and above 50.0 as very severe. Moderate eczema falls between 7.1 and 21.0. Many biologics (e.g. dupilumab) show efficacy when EASI-75 (≥75% reduction from baseline) is achieved as a trial endpoint.
