The APGAR score was developed by anaesthesiologist Virginia Apgar in 1952 as a rapid, standardised method to evaluate a newborn's condition immediately after birth. Each of the five criteria is assigned a score of 0, 1, or 2, giving a maximum total of 10. The five components form a convenient mnemonic: Appearance (skin colour), Pulse (heart rate), Grimace (reflex irritability), Activity (muscle tone), and Respiration (breathing effort).

The score is typically recorded at 1 minute and 5 minutes after birth. The 1-minute score reflects the newborn's tolerance of the birth process; the 5-minute score reflects adaptation to extrauterine life. If the 5-minute score is below 7, additional assessments are made every 5 minutes up to 20 minutes. A score of 7–10 at 5 minutes is considered normal and indicates the infant requires only routine post-delivery care. A score of 4–6 suggests the infant may need some resuscitative measures; a score of 3 or below indicates the need for immediate resuscitation.

Limitations: APGAR is a snapshot, not a predictor of long-term outcomes. Preterm infants often score lower due to physiological immaturity unrelated to distress. Maternal sedation, congenital malformations, and neuromuscular disorders also affect the score independently of acute asphyxia. The APGAR score should always be interpreted alongside other clinical assessments by trained neonatal staff.

An APGAR score of 9 means the newborn is in excellent condition — 4 out of the 5 criteria score 2, with one criterion (usually skin colour, due to mild acrocyanosis — blue hands and feet) scoring 1. A score of 9 is effectively equivalent to a 10 in clinical significance; perfect 10s are uncommon because peripheral cyanosis is normal immediately after birth.

Not necessarily. A low APGAR score indicates the infant needed assistance at the time of the assessment, but it does not reliably predict long-term neurological outcome. Studies show that most infants with a low 1-minute APGAR recover fully with resuscitation. Sustained low scores (≤3 at 5 and 10 minutes) are associated with higher risk of adverse outcomes, but the score alone is not diagnostic of hypoxic-ischaemic encephalopathy.

Virginia Apgar (1909–1974) was an American obstetric anaesthesiologist at Columbia University who devised the score in 1952 to provide a consistent, rapid method for evaluating newborns. The backronym "Appearance, Pulse, Grimace, Activity, Respiration" was created later (by paediatrician Joseph Butterfield in 1962) as a mnemonic — a fortunate coincidence since it also honours its inventor.
